Proposal Preparation (Including Renewal and Supplemental Proposals):

The following are required to be submitted to SPO for review prior to SPO endorsement of a proposal containing a Multi-Campus Agreement (MCA).

• 1. Multi-Campus (MCA) Commitment Form (UCSB/OR SK Form 101) To be completed by Subrecipient, must include Subrecipient's Statement of Work (SOW) and Budget.

 

The following are required to be submitted to SPO for review prior to SPO endorsement of a proposal containing a Subaward.

• 1. Subrecipient Commitment Form (UCSB/OR SK Form 100) To be completed by Subrecipient, must include Subrecipient's Statement of Work (SOW) and Budget.

The following may be required, based upon completion of Subrecipient Commitment form:

• Financial Management Systems Questionnaire
• Certification of Compliance with A-133

• 2. Grant/Cooperative Agreement: Fair & Reasonable Cost Analysis Subawards under Grants (UCSB/OR SK Form 800 Part A-1)  or Contracts: Fair & Reasonable Cost Analysis Subawards under Contracts (UCSB/OR SK Form 800 Part A-2). To be completed by UCSB PI.

The following may be required, based upon completion of Part A-1 or Part A-2:

• Sole Source Justification (UCSB/OR SK Form 200)

Award Stage:

The following is required to issue a subagreement:

• Request to Issue a New or Amended Subaward (UCSB/OR SK Form 800 Part B) To be completed by UCSB PI.

Post Award Stage:

• Subagreement Performance (UCSB/OR SK Form 300) To be completed by UCSB PI and submitted with every subrecipient invoice.

Close-Out Stage:

• Subagreement Close-Out Certification (UCSB/OR Form 330) To be completed by UCSB PI.. Please see "Notice of Subaward for list of required reports".
